HOW & WHO

HOW OUR SERVICES WORK

A Clear and Collaborative Support Journey

We follow a structured service delivery process to ensure that each participant receives appropriate, individualised support

Step 1 — Referral

Submit a referral through our online referral form, by email or by contacting our intake team.

Step 2 — Initial Intake

Our team reviews the referral, confirms eligibility and relevant NDIS funding, and obtains the necessary consent.

Step 3 — Practitioner Allocation

The participant is matched with an appropriate practitioner based on their needs and the required level of clinical expertise.

Step 4 — Assessment

Our practitioner gathers relevant information, conducts observations and completes the required assessment process

Step 5 — Individualised Planning

Based on the assessment findings, appropriate strategies, interventions and support plans are developed.

Step 6 — Implementation

Our team works with the participant and their support network to introduce strategies and provide guidance or training.

Step 7 — Review & Ongoing Support

Progress is monitored, outcomes are reviewed and strategies are adjusted where required.
